Abstract Funding Acknowledgements Type of funding sources: Public hospital(s). Main source(s): University Medical Centre Ljubljana, Slovenia. Background The results studies transcatheter mitral valve repair introduced a new conceptual framework that categorized regurgitation (MR) into proportionate (propMR) and disproportionate (dispropMR) according to the relationship between effective regurgitant orifice area (EROA) left ventricular (LV) end-diastolic volume (EDV). However, pathophysiological mechanisms behind these entities have not been well defined. Purpose To identify echocardiographic characteristics which could explain underlying mechanism dispropMR. Methods We retrospectively included 179 consecutive patients (age: 69±12 years, male: 132 [74%]) with HFrEF more than mild MR referred clinically indicated echocardiography. disproportional cut-off value specific for our group was calculated from theoretical line proportionality based on ratio EROA/EDV, considering clinical study group. Correspondingly, disproportionality 0.126. Potential factors might affect scheme were analysed: affecting EDV measurements (LV foreshortening) EROA calculation (dynamic variation flow rate (MRFR) different PISA approaches calculation: standard single-point method vs. serial method). Results In cohort, 55 (32%) had Patients dispropMR frequent LV foreshortened images propMR (31 (56.4%) 37 (32.7%); p = 0.003), resulting in smaller DispropMR substantial dynamic compared (Figure A), significant overestimation MRFR when using instantaneous (p 0.001). consistently overestimated by B). Consequently, 73% classified as either image or measurement. Conclusion Our suggest higher EROA/EDV be explained underestimation due foreshortening flow. data methodological limitations an important impact used definition proportionality. concept is questionable may support patients’ selection treatment strategies.

Possible Effect of CABG on Moderate Mitral Regurgitation
Introduction: Mitral valve insufficiency in adults is often a complication of ischemic heart disease. Coronary artery bypass grafting (CABG) is performed as a strategy for the treatment of coronary artery disease. The aim of this study was to evaluate mitral regurgitation (MR) before and after CABG. Materials and Methods: This Experimental study was condu...
متن کاملMITRAL REGURGITATION Quantitation of mitral regurgitation by Doppler echocardiography
The evaluation and care of patients with mitral regurgitation would be facilitated by an easy, reproducible and noninvasive method that could quantitate the hemodynamic burden. In this study, we describe a new Doppler echocardiographic method that measures the regurgitant fraction and we compare it with angiographic and scintigraphic methods. A total of 27 patients with mitral regurgitation wer...

Accurate clinical assessment of the severity of mitral regurgitation is often difficult, and left ventricular cine-angiocardiography is probably the most commonly used method of estimating it. It is therefore important to try to determine the accuracy of this method and to identify as far as possible any causes of error. We have attempted to do this in a series of 137 patients submitted to oper...
متن کاملQuantification of Mitral Regurgitation
Background. In clinical color Doppler examinations, mitral regurgitant jets are often observed to impinge on the left atrial wall immediately beyond the mitral valve. In accordance with fluid dynamics theory, we hypothesized that a jet impinging on a wall would lose momentum more rapidly, undergo spatial distortion, and thus have a different observed jet area from that of a free jet with an ide...
متن کاملAssessment of mitral regurgitation.
Mitral regurgitation (MR) is the most commonly encountered valve lesion in modern clinical practice. The range of pathologies producing regurgitant mitral valve dysfunction is broad (table 1) and the condition may be met in virtually any medical speciality. As echocardiography is the most widely available cardiac imaging modality, it is the technique which is routinely used to assess patients w...
